A case of asymmetrical, unilateral chordoma of the head and neck was presented by an 11-year-old girl. Physical examination was unremarkable and the child presented with a persistent, recurring headache. Magnetic resonance imaging revealed a 2-cm, sharply delineated mass in the caudal part of the clivus. The lesion was low on T1-weighted imaging and showed a strong heterogeneous contrast enhancement on T2-weighted imaging. In addition, computed tomography (CT) scans of the head and neck revealed a sharply delineated mass with no bone tissue. The diagnosis was made as a chordoma.

Patients with chordomas of the spine and skull base typically have mutations in tumour suppressor genes.
