Chordoma in Childhood Symptoms - Oren Zarif - Chordoma in Childhood
Children with chordomas often have symptoms that are similar to those seen in adults. Most commonly, they have a tumor that starts in the spinal cord, which can extend into the skull. Before a child is born, they have a thin bar on their back called a notochord. This supports the spine and skull during the pregnancy and disappears before birth. But a genetic change in the notochord's cells causes it to grow too quickly, causing it to 'overgrow' and cause chordomas.

A study published in 2015 showed that a small number of pediatric patients had a chordoma in their heads. The average age at diagnosis was 12 years, and most common presenting symptoms were diplopia, headache, and swallowing difficulties. Although most children with chordomas experience recurrence after surgery, five patients developed locally recurrent tumors. Of these, twelve patients underwent an endoscopic endonasal approach alone.
Another eight patients underwent additional procedures, including a surgical resection. In addition to imaging, computed tomography revealed that the mass was soft tissue, but no bone was visible. Therefore, the tumor was diagnosed as a benign tumor in children.

Various imaging tests are available to diagnose chordoma. CT scan (contrast-enhanced X-rays) is a popular test used to evaluate the affected area. MRI (magnetic resonance imaging) uses powerful magnets and radio waves to produce high-quality images. However, there are a few methods to confirm if a child has chordoma, including a biopsy. This involves removing a small sample of tumor cells and having a doctor look at it under a microscope. Similarly, a blood test and a bone scan are recommended to assess the spread of the tumor.

The diagnosis of chordoma in children is based on multidisciplinary management and multidisciplinary approaches. The first step in treatment is surgery. The goal of the procedure is to reduce the tumor volume and achieve complete resection, while keeping any remaining tissues away from the brain and nervous system. Despite the limited number of pediatric patients with chordoma, the survival rate is still high, ranging from 80% to 90%.

A patient with chordoma in childhood should visit her doctor as soon as possible. Moreover, she should be aware of any signs and symptoms of the disease. The first test to determine the diagnosis is a bone scan. This test will identify the bones that are affected by cancer, and determine the extent of the tumor. If the pain is severe, the pediatrician will recommend a bone MRI. The results of the scan will tell the doctor if the cancer has spread and how much it has spread.

While most chordomas in children have localized symptoms, they are typically very rare. The condition can lead to a range of symptoms, including seizures, hearing loss, and paralysis. The most common symptoms of chordoma in childhood include increased intracranial pressure, headaches, and coccygeal nerve pals. In older children, it may manifest as isolated or multiple headaches. In the early stages, however, the condition should not be treated in isolation.

If the tumor in the neck causes symptoms, it should be removed. Surgical treatment is a major option for this condition. The surgeon can remove the tumor and ensure its complete resection. In many cases, the surgery is successful and the cordoma is removed. If the chordoma is found in the skull, it can be excised via surgery. The surgeon should also remove any nearby structures that might be affected by the tumor.

The most common symptom of chordoma in children is persistent headache. Physical examination is usually normal, but magnetic resonance imaging shows a 2 cm soft-tissue mass in the caudal part of the clivus. The lesion is associated with low signal intensity on T1-weighted imaging and extradural compression of the neuroaxis. A bone scan may also reveal the presence of the tumor or its spread.

A multicenter retrospective study of 40 children with chordomas has shown that 85% of these cases occur spontaneously. In contrast, about 20% of children with chordoma have a tumor on the notochord. An ultrasound can detect the presence of a tumor in a skull. A biopsy can confirm the presence of a chordoma. The surgeon can determine whether a recurrence occurs.
